When needed, Home Care Coordinators are available – We have three full-time Home Care Coordinators that are licensed professionals with more than 15 years of patient coordination experience. Once a referral is made to us here at Care Connection of Cincinnati, a Home Care Coordinator meets with the patient and family at the hospital or rehab facility that they are currently receiving care.

Patient Advocate - This professional position combines patient advocacy with impeccable customer service at the same time addressing any issues in a timely manner. Our patient advocate's responsibilities include assisting patients/caregivers with questions that may arise during the implementation of services they receive from Care Connection of Cincinnati. In addition, she is able to manage patient care through our follow-up process, maintain and update usable data and generate quality reports as needed.
