Making a Difference
Anyone can make a referral to our agency! Our phone lines are staffed 24 hours a day for your convenience at (513) 842-1101. The only information we need is a patient’s name and telephone number, as well as their physician contact information. Then, let us do the rest! We will contact the physician for you and obtain the orders to begin your home care services. And let us reassure you that our services are completely covered by Medicare and Medicaid.

Once a referral has been made, our Intake RN will gather all of the information we need to start the patient’s treatment. She will also obtain any necessary physician orders to begin care.

If your family member or loved one is in a health care facility (hospital, skilled nursing facility, rehabilitation unit, etc.) our professional Home Care Coordinators will meet with you and your family at that location to assist with your discharge arrangements to ensure a smooth transition back home.

Our staff will remain in communication with you from the moment your visits have been authorized by your insurance company or until other financial arrangements have been made.

Your Personal Scheduler will call to make an introduction and let you know which of our nurses, aides and/or therapists have been assigned to your case.

All of your caregivers will also call you to introduce themselves and set up a time to provide your services the evening prior to your visit.
