- Promote and maintain improvements in physical endurance
- Improve the patient’s sense of well-being and independence
- Decrease the number of exacerbations resulting in hospitalizations

- Fall Prevention:
- Striving to increase awareness and knowledge by partnering with Hamilton County Fall Prevention Task Force:
- Educating patients concerning the risk factors and prevention
strategies associated with falling through home health services
- Developing home exercise programs for strength and gait training through Physical and Occupational Therapies
- Assisting patients in understanding appropriate medication compliance (taking four or more required medications significantly increase the risk of falling)
- Informing patients on possible home hazards as well as making recommendations for home modifications

- Personal care assistance for patients struggling with basic daily movements

- Medical Social Services: We are often called out to a home to assess the patient’s in home care needs and recommend other community resources that might prove valuable to the patient and family. They are able to put the patient, as well as family caregivers, in touch with programs that will maximize the patient’s recovery. Social workers may also provide basic counseling to help patients and family members adjust to the demands of at home care.
